COVID-19 Pfizer Vaccine now available for ages 12 and up

Dear UUSD Families:
With the Federal Drug Administration (FDA) and Centers for Disease Control (CDC) authorization of the COVID-19 Pfizer vaccine down to age 12, many of you have contacted Ukiah Unified for information regarding opportunities to get your child vaccinated.
Mendocino County Public Health will be offering vaccination clinics in our schools that serve 12 - 15-year-olds. Please see those opportunities below as well as other available locations and clinics:

- Friday, May 21 at Eagle Peak Middle School from 8 AM - 10 AM for Eagle Peak Middle School students and their families. Please complete the attached Consent to Vaccinate form and return it to your child's teacher by Tuesday. May 18.
- Friday, May 21 at Pomolita Middle School from 11 AM - 1 PM for Pomolita Middle School students and their families. Please complete the attached Consent to Vaccinate form and return it to your child's teacher by Tuesday. May 18.
- Friday, May 21 at Ukiah High School from 2 PM - 5 PM for Ukiah High School students and their families. Please complete the attached Consent to Vaccinate form and return it to your child's teacher by Tuesday. May 18.
- Saturday, May 22 Drive-through Clinic at Ukiah High School. Please click on the link to register and sign up for an

At this time, COVID-19 vaccinations are not required for school attendance, but we wanted to make sure you are aware of the opportunities in our community to have your child receive the COVID-19 vaccine should you choose to do so.

Con la autorización de la Adrninistracion de Drogas y Alirnentos (FDA) y los Centros para el Control de las Enferrnedades (CDC) de la vacuna contra COVID-19 de Pfizer para niños de 12 años de edad, rnuchos de ustedes se han cornunicado con Ukiah Unified para obtener información sobre oportunidades para hacer vacunar a sus niños.
El Departamento de Salud Publica del Condado de Mendocino va a ofrecer clinicas de vacunación en las escuelas donde tenernos estudiantes entre los 12 y los 15 años de edad. A continuación aparece una lista de dichas oportunidades, asi corno otros lugares y clinicas disponibles:
